Accessing your Health Information
Under Ontario’s Personal Health Information Protection Act (PHIPA), you have the right to access your health information. There are two ways to do so securely and at no cost online:

How to Request a Copy of your Patient Record from the MHA
To request a copy of your patient record, first complete the Consent for Access or Disclosure of Personal and/or Personal Health Information form.
Then follow the instruction on either of the below forms depending on which hospital you visited:
and then choose one of the below 3 options:
| (1) Fax it to the Health Records Department at the hospital you visited: Strathroy – 226-330-0933 FCHS – 226-330-0927 | (2) Email it to ROI@mha.tvh.ca | (3) Drop it off in person at our Health Records department at the hospital you visited (SMGH or FCHS). See the wall directories in both hospitals for department locations. |
Note: If you are requesting the health records for someone else, consent or proper legal signing authority will be required.
Please note:
(1) There is a fee for requests for copies of personal health information and we will be able to determine what the amount will be once we have the details of your request.
(2) You will be asked to provide positive identification before the copies of your health record will be released.
